The hospital is the most skilled level of care and is critical for those in life threatening situation. It provides total support for the patient. Admissions arise from emergency situations like accidents or acute medical events or they can be for elective scheduled procedures.
Patients need total help with personal care such as showering, toileting and feeding. Since the hospitals have been forced to cut their staff to a minimum, personal attention can sometimes be compromised. Some patients who can afford it hire private duty nurses or aides to assist them since the nurses on the floor have a lot of responsibility and may not be able to give as much one-to-one care as one might like. Also, private duty nurses and aides can cater to the patient in ways the floor nurse never would be able to in terms of companionship and personal preferences with respect to food and personal care needs.
Many of the elderly are admitted to the hospital due to complications ensuing from their chronic conditions. They often go to the emergency room with a sudden illness and are then admitted for further testing and care. If the situation is terminal, after they are stable enough to be discharged, they may be transferred to end of life palliative care where they are made comfortable during the remaining days of their lives.
